Charlesbank Home Care has a long history of success. While many area home care providers assist those being discharged from hospitals, at Charlesbank, we also specialize in caring for patients who are referred to us from independent living, assisted living and skilled nursing settings, so you can be assured our team is the best prepared to lead you down the road to recovery.

Patients who are discharged from our care have fewer incidents of returning to the hospital or skilled nursing setting. Because we have a large staff and can provide timely access to the therapists and nurses you need… there is no delay or gap in care. As a result, our patients progress faster to recovery or to their prior level of functioning.
