Duties:

As part of the Admissions department hospital inpatient staff, the hospice case manager (HCM) conducts proactive case management for patients while hospitalized. The HCM conducts ongoing clinical nursing assessment, updates the Plan of Care as necessary and coordinates and facilitates timely discharge planning for Hospice patients. Visits to company hospitalized patients on a regular basis and participating in coordination of the inpatient care are necessary.

Qualifications:

* RN (BSN preferred) with current FL License.

* Case Management certification/experience required.

* Minimum 5 years clinical experience with recent acute care hospital experience preferred. Strong CM knowledge and physical assessment skills required.

* Valid driver license and auto insurance per company policy.
