Consonus therapy department focuses on a variety of programs including Stroke Recovery, Orthopedic Rehab, Contracture Management, Cognitive Retraining, and Prosthetic/Orthotic Training, our multiple-systems approach including aquatic therapy, resistance training and our newly-introduced, innovative ACP equipment. Many of our therapy departments offer in and out patient programs as well as wellness programs and weight training and yoga classes. This is considered a sub-acute long term care rehab. A Consonus therapist services the geriatric population. Our therapy departments are usually in a long term care facilities or large retirement campus.
Job Description
Ensure that an evaluation/up-to-date plan of treatment is in the chart prior to initiating treatment. Provide treatment appropriate to evaluation findings and within the professional scope of practice and individual level of expertise. Update report changes in status, function, or medical condition to the supervising therapist. Communicate regularly with other team members and on call therapists. Will obtain co-signature on discharge summaries or end of the month progress reports and on screening forms. Coordinate scheduling issues with team members and the staffing coordinator as needed. Responsible for completing all required documentation in a timely manner (to include reports, daily progress notes, discharge reports, monthly reports, daily service and labor logs, screening logs, forms and other documentation). Responsible for maintaining all continue education requirements and update licensed at all times.
Consonus Rehab Services is a contract therapy company that sets up and runs therapy departments in skilled nursing facilities/long term campuses. These therapy departments can be in and out patient programs. Our employees can find full time, part time or on call/per diem positions in a variety of locations; California, Texas, Idaho, Oregon, Washington, Wisconsin and Nevada.
