Job Description:  Position Summary:

Responsible for conducting and coordinating completion, collaborating with all disciplines, of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) assessment and all associated processes. The Coordinator signs and attests to completion of the assessment within the timeframe mandated by federal regulations and company standards. This person is also responsible for the coordination of the resident care planning processes.

Education and Experience:

• Education: Graduation from a School of Nursing for Registered Nurses.
• Work Experience: One to three years of previous experience in an MDS Coordinator role preferred.
• Licensure / Certification, where required: Current State Licensure as a Registered Nurse in the state where practicing. Cardiopulmonary resuscitation/ Automatic External Defibrillator certification. Food handlers certification, where required.

Principal Accountabilities / Essential Job Functions:

• Validates that participants in the assessment process complete an accurate and comprehensive assessment.
• Assure the insights of non-licensed persons who regularly provide direct care to the residents are included in the assessment process.
• Encourages personnel to report changes in the resident's status.
• Assure that residents and their families are actively involved in the information sharing and decision making process.
• Manages the coordination of RUG scores with ICD-9 coding for Medicare billing.
• Completes Resident Assessment Protocol documents.
• Coordinates the completion of the resident care plan and the care plan conferences per requirements.
• Adheres to the operating budget for the care venue.
