POSITION SUMMARY
 
The Community Education Liaison provides education about Agency services within the assigned geographical areas.
 
ORGANIZATIONAL STRUCTURE
 
The Community Education Liaison reports to the Director of Community Education or the applicable Branch Manager/Director of Nursing.
 
RESPONSIBILITIES
 
1.      Coordinates patient transition from physicians, discharge planners, and care managers into home care settings.
2.      Provides education about Home Health services.
3.      Assures that Agency policies and procedures are enforced and followed by the community.
4.      Demonstrates consistent and fair team skills without favoritism. Demonstrates ability to organize, prioritize, motivate and delegate appropriately.
5.      Serves as a role model to community by performing responsibilities professionally, timely, efficiently and correctly.
6.      Provides appropriate orientation, training and education to community.
7.      Performs other duties as assigned.
 
POSITION QUALIFICATIONS
 
1.      Registered Nurse or Licensed Practical Nurse Graduate of an accredited school of nursing with a valid and current license in the State of Tennessee or equivalent healthcare background with significant marketing experience.
2.      Knowledge of Medicare, TennCare, state and federal regulations and accreditation standards as they relate to home care.
3.      Exhibits professional appearance and behavior.
4.      Proficient in verbal and written English grammar skills
5.      Basic knowledge of use of computer.
6.      Must pass criminal background check and previous employer checks.
7.      Current driver’s license, acceptable driving record and valid automobile liability insurance.
