* FULL-TIME TO COVER FAIRFIELD & SURROUNDING COUNTIES

* PER DIEM FOR JACKSON

* REQUIRED: MED-SURG & HOMECARE EXPERIENCE WITH KNOWLEDGE OF OASIS

POSITION SUMMARY :

The Registered Nurse (RN) Case Manager is responsible for managing the care plans of the patients from admission through discharge and for maintaining the delivery of quality patient care.   The position combines the activities of direct and indirect patient care and directs staff nurses, LVNs, CHHAs in the delivery of the individual patients’ plan of care as well as identifying interdisciplinary needs and coordination of the allied health clinicians.

POSITION QUALIFICATIONS:

This position requires that the individual possess a current Ohio license as a registered nurse, have a minimum of one year of experience as an RN, and sufficient verbal and communication skills to perform required tasks. Current CPR Certification required.

Ability to maintain assigned work hours:   Has sufficient endurance to perform tasks over long periods of work hours.   Ability to travel to all business locations.
