The LVN, GWYN Team provides skilled nursing care to SDH patients in accordance with the treatment and plan of care dictated by patient, family, physician, and members of the interdisciplinary team and under the direction of the registered nurse. The LVN, GWYN Team will provide staffing support to Access Center, Homecare, ICC and CCC to cover intermittent visits, crisis care, or assist with patient care in ICC or CCC as needed. LVN, GWYN Team is expected to "Go Where You're Needed" as a member of this team. Employee may be expected to learn and utilize a window compliant computer system dependent on the frequency of their work schedule. Proficiency and appropriate utilization is a requirement of the job role.
Education: Must be a graduate of an approved LPN/LVN school of professional nursing, who is currently licensed as a Vocational Nurse by the State of California. Must possess a valid CPR certification. Bilingual abilities preferred.
Experience: A minimum of one year LPN/LVN experience recommended. Previous acute care, hospice or home health experience preferred. Good knowledge of home health care principles and practices within the scope of palliative medicine. Familiarity with interpersonal dynamics operative within a health care team and knowledge of the dying and bereavement processes. Possess basic knowledge or aptitude to operate a computer, knowledge of basic software programs is preferred. Must possess excellent assessment, excellent oral and written communication skills, superb organizational skills and ability to prioritize multiple tasks. IV certification and/or Blood Draw certification preferred. Excellent phlebotomy skills a strong plus. Current California Driver's license and automobile insurance required.
