Job Description: 
  • The CHHA/CNA, GWYN (Go Where You're Needed) Team, under the supervision of a Registered Nurse, participates as a support member of the interdisciplinary team by assisting in the provision of a variety of personal hygiene services to SDH patients. The CHHA/CNA, GWYN Team will provide staffing support to Access Center, Homecare, ICC and CCC to cover intermittent visits, crisis care, or assist with patient care in ICC or CCC as needed.   CHHA/CNA, GWYN Team is expected to "Go Where You're Needed" as a member of this team .

    Education: Requires successful completion of the California State Home Health Aide Certification Program that meets the requirements of § 484.36 (a) and a competency evaluation or licensing program that meets the requirements of § 484.36 (b) or (e).   An individual is not considered to have completed a training and competency program, or a competency evaluation program if, since the individual's most recent completion of this program(s), there has been a continuous period of 24 consecutive months during none of which the individual furnished services described in § 409.40 for compensation.   In addition to the aforementioned requirements, CHHA incumbents must have Certified Nurse Assistant card (CNA certificate).   Must possess a valid CPR certification.   Bilingual abilities preferred.

     

    Experience: Requires understanding of medical terminology.   Must complete 12 hours of in-service education each year for CHHA certificate and 24 contact hours to maintain CNA certification.   Must possess personal qualities consistent with understanding of curative and palliative care and End of Life issues and be able to deal with the demands of the job.   Follows directions effectively and possesses good communication skills.   Demonstrates responsibility for self-development.   Must be willing and able to travel to patients' homes/SNFs/inpatient facility as well as the ability to read a map.   For continuous care must have a flexible schedule and willingness to be called onto a continuous care shift with little or no notice.   Current California Driver's license and automobile insurance required.
