Job Description: 
Seeking a RN Case Manager for Home Health Care. This nurse will coordinate the overall plan of care for a group of patients in conjunction with the interdisciplinary team. These nurses will work independently and be responsible for managing a caseload of patients in the area.
Job Requirements:
Graduate of an accredited school of professional nursing, BSN preferred.
Current RN licensure by the State Board in DE.
Two years of general nursing experience preferred to include current experience and at least one of the following: home care, med-surg , or critical care.
IV skills and/or certification preferred.
Excellent nursing judgment, assessment skills. and documentation skills.
Demonstrated clinical competency in applying principles, methods and equipment used in nursing and appropriate specialty.
Identified interest to work in home care.
Current CPR certification. Ability to function as an effective member of an integrated healthcare team.
Licensed driver with a reliable automobile, which is insured in accordance with organization policy.
Will consider applicants lacking home care experience who are eager  to learn a new practice area and possess a strong desire to be part of a growing team of home care professionals.
