Job Description: 
  • The RN, Hospital Liaison performs initial nursing intake assessment of potential SDHIPM patients in assigned hospitals to determine the clinical appropriateness of their acceptance for service and communicates findings to the Interdisciplinary Team in an accurate and timely manner.   Also develops collaborative relationships, educates, and provides excellent service to the customer (patient and family, physician, physician agent, discharge planner, case manager, social worker, Skilled Nursing Facility Administration, and Health Care Professionals) within assigned hospitals.   Employee is expected to learn and utilize a windows compliant computer system. Proficiency and appropriate utilization is a requirement of the job role.

    Various Shifts Available:
               Mon – Fri: 8:30 am – 5:00 pm + weekend shift rotation;
               Tues - Sat: 8:30 am - 5:00 pm;
               Mon - Fri:  11:00 am - 7:30 pm;
               Fri - Sun: 8:00 am - 8:30 pm (12 hour shifts)

    Education: Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California .   Requires a bachelor's or Associates degree in nursing from a program accredited by the National League of Nursing and a Public Health Nurse credential is desirable.   Must possess a valid CPR certification.

     

    Experience: Requires a minimum of two years of nursing experience in an acute hospital-based setting and a minimum of one year in a home health care setting.   Must have an understanding of the principles and practices of home care nursing, with specific knowledge of principles of nursing care within the scope of palliative medicine.   Requires knowledge and understanding of insurance coverage.   Requires a basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary health care team, and knowledge of the dying and bereavement processes.   Possess knowledge or aptitude to operate a computer, knowledge of electronic medical charting is preferred.   Current California Driver's license and automobile insurance required.
