The Transitional Care Director supports occupancy and revenue through the development and management of hospital partnerships and post-acute networks within an assigned market area or region. This position will focus on increasing occupancy, reducing hospital readmission rates, and developing and managing resident/patient care within preferred provider network.

Position requirements include:

  • Bachelor's degree in Nursing and/or Business/Communications or related field preferred
  • Two years sales experience with proven track record in a Medicare-certified organization preferred
  • Experience as a hospital discharge planner preferred
  • Experience in nursing home, or assisted living industry preferred
  • Working knowledge of computers including Excel, Word, and MS Office is required
  • Must meet all health requirements, including TB/freedom from communicable disease
  • Must have valid driver license
  • 80% travel required between regional communities and preferred hospitals
  • Must pass criminal background check and nursing licensure validation as required by state law (if applicable)
  • Must have compassion for and desire to work with the senior population
