Job Description: 

BEREAVEMENT COUNSELOR / PALLIATIVE CARE SOCIAL WORKER
Plans, coordinates and supervises all aspects of age appropriate bereavement services including counseling for individuals and groups, bereavement volunteer training, educational programs related to bereavement issues, development of liaisons with community partners and program development. Provides individual counseling as a licensed therapist. Member of the Palliative Care Consult Pathways program, skilled in helping patients and their support system explore goals of care. 
Master’s degree in social work with LCSW credential required.
