SUMMARY: Serves as a resource for referral sources on services provided, third party regulations, and assist with identifying patients who can manage safely in their home with intermittent services and/or continuous home care services. Provides resource information if patient/client is not appropriate for home care. Obtain accurate information in order to facilitate the gathering of information from referral sources in a timely, efficient manner. Act as a resource to referral sources and the public.

QUALIFICATIONS:        Registered Nurse. Minimum of two years Visiting Nurse experience. Excellent communication skills, verbal and written.        Helpful attitude. Present a positive, professional image of the agency. Computer knowledge and skills.
