SUMMARY:  A clinical staff member who is credentialed to provide expertise in the advances in the disease management of diabetes, can be a critical resource to others, and works to continually improve the quality of care being provided.  Ensures the quality and appropriateness of care being provided to diabetic patients.  Provide consultative services to clinical staff.  Work with physicians and referral sources.  

QUALIFICATIONS:

  •   Current professional licensure as a registered nurse in the States of   Massachusetts and New Hampshire.
  •   Evidence of formal education/certification in Diabetic Education.
  •   Practical experience in diabetic management/education, preferably in home health care.
