An new opportunity is available for a Clinical Liaison Nurse!
This position is full-time and will work between our Rydal Park and Broomal Presbyterian Village communities.

The Clinical Liaison Nurse is responsible for creating and fostering positive relationships with all community referral sources and potential admissions into one of Presby’s inspired life communities. Referral sources include but are not limited to hospital discharge planner, social workers, physicians, nurse practitioners, hospitalists, case managers, and home care agencies. The clinical liaison nurse will work in conjunction with the admissions/marketing director to increase our referral base and help to maintain and grow census.

 

Essential duties:

1.        Manages referral source relationships and supports the implementation of the organization’s marketing plan.

2.        Conducts regular visits to referring hospitals.   Builds and maintains relationships with discharge planners, case managers, physicians, and other referral sources

3.        Liaisons with hospitalized patients to ensure placement in the Presby continuum of care whenever appropriate.

4.        Reviews documentation of potential new admissions, conducting in-person assessments as needed to determine appropriateness.

5.        Maintains thorough knowledge of the market in general, competitors, and acuity trends.

6.        Coordinates evens to promote with the admissions director for all referral sources

7.        Assists in meeting the referral and admissions goals for assigned hospitals

8.        Coordinates tours at the communities for new or existing referral sources as appropriate.

9.        Expands the number of referral sources in the market

10.    Active participation in professional and community relations

11.    Maintains current referral sources through relationship development

12.    Keeps accurate records and creates reports of activities and outcomes weekly

13.    Establish strong and successful relationships with referral sources throughout the area.

Required Experience: Current PA state RN or LPN licensure.  A minimum of 3 years equivalent experience in a long-term care setting.
