RN Case Manager or Per Diem Visiting Nurse
As the RN Case Manager, you will provide professional nursing services to an assigned group of patients and families in the home setting using point of care software for laptop based documentation. You will be responsible for ensuring along with an interdisciplinary team that patients and families are provided excellent care in Santa Clara County County.

Position Type: Full-time, Part-time or Per Diem depending on your location of interest.

Requirements:
› All positions require 1-year recent home health and/or hospice experience.
› All applicants must have a current CPR and state issued license.
