The Supervisor, ICC collaborates and coordinates with the Clinical Manager, ICC to oversee the daily operations of the Unit. Primary duties include the direct supervision of a specified number of department staff and the orientation and training of new staff under the guidance of the Clinical Manager, ICC. Provides administrative and operational support to the Clinical Manager, ICC by performing a variety of functions: product analysis, generating reports and statistics, ensuring regulatory compliance, monitoring quality outcomes, chart audits, and project management assignments as directed by the Clinical Manager, ICC. Collaborates with the Clinical Manager, ICC to hire, retain, counsel, discipline and terminate staff. Responsible for the daily staffing to ensure coverage for all patients. Serves as a clinical expert in the ICC and assists the Clinical Manager, ICC with the identification and implementation of process improvement projects and department education. Supervisor is expected to learn and utilize a windows compliant computer system. Proficiency and appropriate utilization is a requirement of the job.
Education: Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California. BSN preferred. Must possess a valid CPR certification. Bilingual abilities preferred.
Duties & Responsibilities:
· Collaborates with the Clinical Manager, ICC to hire, retain, counsel, discipline and terminate staff
· Ensures that staff use appropriate clinical knowledge to: assess patient and family issues, perform and document skilled needs and follow plan of care (POC)
· Facilitates and participates in meetings when designated by the Clinical Manager, ICC
· Collaborates on the day-to-day operational aspects of the ICC with the Clinical Manager, ICC. Assesses program needs and communicates them to the Clinical Manger, ICC
· Assists with data collection and operationalizes ICC goals as appropriate
· Provides clear accountabilities and individual coaching for subordinate staff. Requires on-call support coverage as needed or requested
· Functions as expert resource for the ICC
· Identifies and develops preceptors to assist in the orientation of new employees
· May serve as preceptor for new employees
· Makes recommendations for developing and/or revising care standards and upgrading clinical performance
· Provides in-services as needed regarding administrative, programmatic or operational goals of the department/agency
· May assist with nursing functions to support the team, including but are not limited to patient admissions, transcribing orders, assisting with patient discharge
· Communicates effectively with ICC staff and other agency staff
· Identifies and consults with resources within agency and/or the community to meet the needs of the patient and family/caregivers
· Actively participates in coordinating, developing, and implementing internal educational in-services
· Assists Clinical Manager, ICC in achieving established departmental goals, the identification and implementation of process improvement projects and departmental education.
· Knows and incorporates evidence based clinical practice and procedures
· Initiates and implements quality improvement activities
· Directly confronts issues that impact quality of care
· Develops and/or revises policies, procedures, and care standards
· Assist with coordination and maintenance of the ICC quality measures and outcomes
· Ensures all ICC staff function within regulatory compliance in areas not limited to: documentation, clinical standards, policies, and applicable laws
· Responsible to participate, enforce, and support the ICC and SDHIPM organizational compliance activities and take appropriate action in reporting issues related to compliance
· Follow policies, established standards and guidelines as may relate to synchronization, accessing data, documentation and equipment/data use and protection
Experience: Requires a minimum of five years experience in their area of discipline, hospice or home health and have demonstrated prior supervisory or leadership experience. Must have an understanding of the principles, practices, and regulatory requirements of the acute care setting and hospice care. Must demonstrate critical thinking and problem solving skills using evidence based practices and to work independently with a minimum direction to achieve expected results. Requires strong analytical, organizational and interpersonal skills with high attention to detail. Requires an expert understanding of interdisciplinary health care teams, and knowledge of the dying and bereavement processes. Possess basic knowledge or aptitude to operate a computer, knowledge of basic software programs is preferred. Requires excellent written and verbal communication skills. Presentation and training skills required.
