  • Creates, generates and distributes statements of medical necessity (SMN's) for patients, obtains signatures from physicians for claim submission.
  • Assembles and maintains patient charts to file appropriate documentation, including SMN's, AOB's (patient agreements), POT's and prescriptions.
  • Maintains a tickler systems for all forms required for billing function.
  • Tracks missing forms via phone, computer and manual logs and computer generated tracking reports. Contacts appropriate branch personnel if problems occur.
  • Performs other related duties as directed by supervisor.

Qualifications:
  • Education or experience equivalent to a high school diploma is required.
  • At least one year related experience, with previous customer service and/or some experience with computers is preferred.
