Job Description: 

Responsible for the delivery of care and services to the resident within the assigned area during a given period of time. Delivery of resident care is performed through the identification of resident needs and prescribed interventions.

Performs clinical and technical aspects of care, collects and evaluates data about each resident's care, and develops a service plan based on resident needs and resident requests for support with activities of daily living.

Education: Graduation from a School of Nursing for Licensed Practical/Vocational Nurses.
Work Experience: Prior experience working with geriatric clients preferred.
