Health at Home™, an LCS company, has an immediate opening and an exciting opportunity for a Register Nurse with a customer service focus .  The RN Case Manager is a unique and challenging opportunity for a skilled professional with a sincere interest in providing care in a geriatric setting. 

Ideal candidates will have experience in a Medicare certified program , be detail oriented, a self-starter, and have great communication skills .  Organization is essential along with a focus on customer service and problem solving and decision making skills.

Required Skills
  • Current Connecticut RN license
  • Ability to read, write, and communicate in English. 
  • Requires effective interpersonal skills.  Must have excellent organizational skills and problem solving abilities.
  • Must be a team player, flexible and able to handle multiple tasks. 
  • Strong customer service and interpersonal communication skills.
Required Experience
  • Minimum 1-2 years work experience as a Registered Nurse in a home health, hospital or health care facility.
