Job Description: 

Girling Health Care's Home Health division is currently seeking a Fee For Service Social Worker in Humble, TX!

 

Assesses the social and emotional factors in order to estimate the patient's capacity and potential to cope with problems of daily living. Helps the patient/family/caregiver to understand, accept, and follow medical recommendations and provide services planned to restore the patient to optimum social and health adjustment within his/her capacity. Assists patient/family/caregiver with personal and environmental difficulties with predispose toward illness or interfere with obtaining maximum benefits from medical care. Utilizes resources, such as family and community agencies, to assist patients to resume life in the community or to learn to live within their disability. Assists the physician and other members of the health care team in understanding significant social and emotional factors related to the patient's health problems. Complies with all applicable rules and regulations, Agency policies, procedures, and processes.

 

This position will service the Coldsprings, Huntsville and Livingston areas.

Required Experience

Qualifications:

  • Free from Medicare Sanctions (required)
  • Master's Degree from a School of Social Work Accredited by the Council on Social Work Education (required)
  • Current State License where required
  • CPR certification required within three months of hire
  • Minimum one year social work experience in a health care setting (required)
  • Adequate means of transportation to facilitate timely visits (required)
  • One year of Home Health experience (preferred)
