Job Description: 
Provides full range of hospice nursing care to patients and families during a crisis period for a patient/family. Works under the supervision of a hospice team RN and the Continuous Care manager. Monitors patients' medical condition over extended periods of time and provides accurate reports to the patients RN and Continuous Care manager. Works independently in patients home care setting providing 1:1 patient care to teach, assist, and support patient/family during the time of crisis. Due to business needs, may be required to work different shifts, teams and location as assigned.
Requirements
Current Florida LPN license; 2-3 years nursing experience preferably in oncology, palliative care, geriatrics, home health care or the private duty sector. Successful completion of IV therapy course (30 hours). Demonstrated ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles. Experience working with interdisciplinary team preferred. Current Florida driver's license and insurance required.
