Girling Health Care's Home Health division is looking for a Fee-For-Service Licensed Physical Therapist in the DeWitt, IA area. Dubuque, Jones, Jackson, Clinton, Cedar, Scott, Muscatine Counties. Quad Cities (Davenport) area.
Description:
Conducts a thorough and precise assessment of the patient's condition, functional abilities, homebound status, and environmental factors. Makes appropriate recommendations for necessary equipment, home modifications, and referrals to PT facilities and other therapies as indicated. Provides care in accordance with physician orders, the patient's condition, accepted standards of practice, Agency policies, procedures, and processes. Identifies changes in the patient's condition, consults with physician and/or Patient Care Coordinator (PCC) and implements appropriate interventions. Utilizes appropriate infection control techniques. Instructs patient, family and or/caregiver in all aspects of patient care. Participates in care planning. Develops and supervises Physical Therapy Assistant (PTA) and/or Home Health Aide (HHA).
Qualifications:

Current PT License (required) Current CPR certification (required)
Adequate means of transportation to facilitate timely visits (required)
Minimum of 12 months experience as a PT, with Home Health experience (preferred)
