MEDICAL SOCIAL WORKER, HOME CARE SERVICES

Position Summary
Assess and develops a plan of care to assist in meeting the psychosocial needs of patients to enable them to achieve their optimum level of health and psychological functioning. Assists families of patients in providing support necessary to maintain their relative at home. Interacts with adults and the geriatric population.
General responsibilities include: 1.) Interviews patients/families on admission to obtain mental, social, environmental and financial information; 2.) Provides home visits determined by the plan of care in collaboration with primary nurse according to physicians orders; 3.) Collaborates with outside agencies to promote well being of the elderly population and disable in community or hospital setting; and 4.) Collaborates with other disciplines and community agencies in assessing and managing patients’ psychosocial needs
Position Qualifications
Education: MSW from a graduate school accredited by the Council of Social Work Education and the FL Education Department
Licensure: Must have current and valid Florida State driver’s license, auto liability insurance, and ability to provide own transportation for home visits. Motor vehicle record must be acceptable to BSSPHC insurers and meet BSSPHC policy standards. Must have CPR.
Experience: 2-3 years social work experience with the adult disabled and elderly. One year in health care setting preferred.
