Location: Kate B. Reynolds Hospice Home Position Type: Full-Time Schedule: 7:00pm - 7:00am Shift: Nights Position Requirements: Current NC license as a Registered Nurse and current CPR certification. A minimum of two (2) years' clinical experience preferred. Possess good nursing assessment and problem solving skills. Ability to work well within an interdisciplinary team setting as well as independently with a minimum of supervision. Possess excellent interpersonal communication skills, both oral and written. Possess excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks. Ability to work a flexible schedule. Position Duties and Responsibilities: A Registered Nurse (RN) assigned to patients and their families has responsibility for assessing their needs and for developing, implementing, and evaluating a plan of nursing care. The RN will provide direct patient care, in accordance with the Nurse Practice Act and Hospice and Home Health standards of care. The RN will understand and adhere to Hospice and Palliative CareCenter (HPCC) policies and procedures and be an advocate of the Hospice philosophy. Provide nursing services according to the plan of care. Communicate with physicians and other members of the Interdisciplinary team as needed to coordinate and ensure high quality care. Develop a plan of care with goals defined by the patient/family. Supervise care provided by Nursing Assistants I and II and Licensed Practical Nurses. Provide personal care in the absence of NA or in conjunction with the NA. Ensure clean and sanitary environment for patients. Attend and participate in Interdisciplinary Team (IDT) meetings, In-services, staff meetings and daily report. Provide ongoing education to patient and family. Complete clinical documentation according to agency, state, federal and accreditation standards in an accurate and timely manner. Adheres to state and federal accreditation regulations and agency policy and procedures. Provides quality customer service to patients, families, co-workers, supervisors and visitors. Establishes and fosters positive working relationships. Represents the agency in an appropriate manner at all times. Perform other duties as assigned by supervisor or CEO.
