Job Description:  Plans, directs and coordinates the hospice clinical team program, including home care, inpatient care and nursing home/ACLF program. Works in an interdisciplinary care setting to teach and mentor patient care personnel. Coordinates all interdisciplinary functions and ensures that patient and families needs are met. Participates in the on-call rotation as necessary. RN with BSN preferred. 3-5 years supervisory or leadership experience in a healthcare setting with oncology, hospice, home care experience. Must be skilled in coordinating an interdisciplinary team concept of care and have excellent interpersonal, communication, problem solving and leadership skills.
