Job Description:  Works with patients and families in providing skilled nursing service, performs initial and ongoing nursing assessments, develops plan of care per physicians plan of treatment, evaluates patients response to treatment, maintains and applies knowledge of Medicare/Medicaid regulations, documents care given in patient record, maintains medical records per regulations, coordinates care, supervises Home Care Aides, and manages patient case load. Prefer a candidate with knowledge of infusion delivery systems, infusion skills a priority; skilled in peripheral and midline insertions. Attends 15 hours of in-services/year. Requirements Must have a current RN license in appropriate state (MO/KS), with a minimum of one-year (1) home health experience. Must have a current Driver’s License and proof of auto insurance.
